Ephedrine are fine, white, odorless crystals or a powder with a molecular weight of 165.234 and a melting point of 187-188°C. It is a sympathomimetic amine and a structural methamphetamine analogue, differing from methamphetamine only by the presence of a hydroxyl group (—OH). Ephedrine is commonly marketed as the hydrochloride or sulfate salt and is used to manage and treat clinically significant hypotension.

Medical uses of ephedrine

Ephedrine is a medication with various medical uses, including:

  1. Treatment of Hypotension: Ephedrine injection is used to manage low blood pressure caused by anesthesia
  2. Asthma and Allergic Conditions: Ephedrine is indicated for the treatment of allergic disorders like bronchial asthma
  3. Narcolepsy and Obesity: While not the preferred treatment, ephedrine has been used for narcolepsy and obesity
  4. Nasal Congestion: Ephedrine has been used for nasal congestion, although its benefit in this regard is unclear

Ephedrine works by increasing the activity of alpha and beta adrenergic receptors, leading to effects similar to epinephrine, such as increased blood pressure, heart rate, and contractility. It can be administered intravenously, subcutaneously, intramuscularly, or orally, with varying onset times and durations of action depending on the route of administration.

Additionally, ephedrine is a sympathomimetic amine with cardiovascular effects similar to epinephrine and is available in various forms under different brand names like Akovaz, Bronkaid, Corphedra, Emerphed, Primatene, and Rezipres.

Contraindications for using ephedrine include:

  1. Closed-Angle Glaucoma: Ephedrine is contraindicated in individuals with closed-angle glaucoma
  2. Hypersensitivity: Patients with hypersensitivity to ephedrine or other stimulants should not use ephedrine
  3. Cardiovascular Conditions: Individuals with conditions like tachyarrhythmias, ventricular fibrillation, hypertension, and cardiovascular disease should avoid ephedrine use
  4. Pregnancy and Breastfeeding: Ephedrine should not be used during pregnancy unless specifically indicated by a qualified physician. Its use during breastfeeding is not recommended.

Additionally, ephedrine should be used with caution in patients with inadequate fluid replacement, impaired adrenal function, hypoxia, hypercapnia, acidosis, hyperthyroidism, prostatic hypertrophy, diabetes mellitus, and during delivery if maternal blood pressure is high.

It is also contraindicated in conditions like phaeochromocytoma and asymmetric septal hypertrophy (idiopathic hypertrophic subaortic stenosis)

Prior to using ephedrine, it is crucial for healthcare providers to assess the patient’s medical history and consider potential drug interactions to ensure safe and effective treatment
